I warrant that my child is in good health and has no physical condition that would prevent participation in activities at CAPE FUR SEALS GIRLS FOOTBALL ACADEMY. I agree to instruct my child to follow all academy rules and regulations, including dress and safety codes at all times, and that my child’s failure to adhere to the rules and regulations may result in my child being removed from activities offered by the Academy presently and permanently without a refund of any fees. I herewith confirm to have understood the Terms and Conditions and that I agree and consent to the conditions contained therein, including but not limited to the Code of Conduct. I hereby grant consent to the instructors and/or supervisors of the academy to seek medical aid for my child in the case of an emergency. I acknowledge that it is my responsibility to secure appropriate medical insurance and no such coverage is provided or implied by CAPE FUR SEALS GIRLS ACADEMY.Agree
